How To Use

Whilst the forms are simple to use, you need to know a couple of important points.
Once you start a form you will have to complete it, there is no save function.
These forms are completely Unique and copyrighted to Im Online, they have been designed by us from our experiences with the NDIS.

Which Forms To Use

The forms you require are dependent on where in the NDIS Process you are.
If you are in the process of applying to the NDIS, then when you purchase our subscription NDIS Access Request Forms & Resources, the appropriate forms will be listed.

Form Output

All our forms when you click on the Submit Button will be emailed to you in "Microsoft Word" format.
This allows you to change or update the information as required.
You can also click on the "Download Button" on the Thank You page.

Check List

The Check List is a way in which to ensure that you have all the documentation, information, etc required to be provided

Impact Statement

An Impact Statement is one of those documents that there are divided thoughts over. Some organisations say don't bother, they don't read them. This can be a very powerful document. Otherwise how can the NDIA understand the full situation, without a complete picture.

Medical Information

A clear perspective of the participants current Medical Situation. Enhanced when completed in-conjunction with a Medical Professional. The Disability is only part of the big picture. Combined with the Subsequent Conditions tells the Actual Picture.

Initial Plan

It's important to present a full picture of the participant at your Initial Plan Meeting. This can be achieved through building a physical, mental, emotional, health and functional picture on a daily basis, inconjunction with their future goals and dreams.

Functional Limitations

A picture of the participants Functional Ability, in everyday life. How a Disability combined with Subsequent Conditions affect your Functional Abilities on a daily basis.

Milestones Under 7 Years Of Age

How many times throughout your children's life, have you been asked Milestone questions. Unless you have kept detailed records, or have a file system, with all information kept within it, these questions become estimates or as we call them "guestimates". This form allows you create a journal about your child whilst the memories are fresh.

Detailed Daily Supports

This layout details out the level of actual Informal and Formal Support being accessed. It is important for the NDIS to have a complete overview of their level and types of supports being utilised plus the ones required in the future. Without this information you may be under funded and be in a situation of not being able to afford the level of support going forward.

Plan Reassessment

The NDIS now calls the "Plan Review" a "Plan Reassessment". You are required to complete a NDIS form for this type of Meeting. To ensure that you capture all the information we have created an Attachment Form to be completed in conjunction with the NDIS Form.

Estimated Costings

The Estimated Costings allows you to list out all the services, support providers and workers, Consumables, Low Cost Assistive Technology, etc, with either hours or dollar amounts. It also enables you to list the types of reports and assessments you expect to require within the span of the new plan.

Primary Carer

Quite often the health and wellbeing of the Primary Carer is taken for granted. The Carers Health and Wellbeing is important and the NDIS needs to understand the Carers position.
